Care Navigation Pathway

Not all health problems need to be managed by a GP. All health enquiries will pass through our care navigation process. Care navigation enables our staff to ensure you receive the right care, in the right time, by the right health care practitioner. When accessing our services, our trained Care Navigators will need to ask you a series of questions about your enquiry to determine the most appropriate pathway for it. Our Care Navigators have undertaken training in line with our practices agreed protocols. Care Navigation staff, like all members of the team, are bound by confidentiality rules Any information given by you is treated strictly confidentially. The Practice would take any breach of confidentiality very seriously.

UPDATE  June/July 2022

We will be commencing the administration of the Covid-19 spring boosters on behalf of Severnvale PCN. We will be holding the clinics in the Vaccination POD and running them Mon – Fri. Invites will start to be sent in due course. We will be inviting over 75’s and those in the category of immunosuppressed. Please wait to be invited.

The government and NHS England have asked us to focus on vaccinating in the coming weeks,  We appreciate your patience. As always, our team will be working hard to prioritise those who are most in need. Thank you for your understanding.

If you have already received a text message or letter from the surgery about booking your vaccine, please use the link in the text message to make your appointment.

If we haven’t yet been in touch, you will hear from the surgery when we have an appointment available. We are working hard to get our appointments set up, so this should be very soon. In the meantime, please do not contact us about getting a vaccination appointment.

How to book

Everyone eligible can book a vaccine appointment online. You can do this here. You can also get your vaccination at a walk in centre. Full details of walk-in clinics and how to access them are available here (you may need to book an appointment in advance).

  • Keep an eye on our website and social media for updates
  • Wait for us to contact you – you will get a letter or text message

Third dose vaccination for severely immunosuppressed aged 12+

The JCVI has advised that people with severely weakened immune systems should have a third COVID-19 vaccine dose.  This is separate to any potential COVID-19 booster vaccination.  There are national recommendations as to who is considered eligible for the third vaccine and General Practices are working with Hospital Specialists locally to identify those who should receive this.  There is no need contact the NHS.  If you or your child is eligible for vaccination, we will contact you.

For more information see  JCVI issues advice on third dose vaccination for severely immunosuppressed – GOV.UK (www.gov.uk)

MASKS

Everyone accessing or visiting the practice must continue to wear a face covering and follow social distancing rules at all times.

Covid 19 rates are very high locally and the threat of the virus remains. We ask that everyone does their bit to help and minimise the spread of infection.

These measures remain in place to protect our patients and our staff, so that the most vulnerable among us can continue to safely attend healthcare settings.

PRACTICE

The practice is open and will continue to offer a range of telephone, video and face to face appointments.

Staff are working extremely hard to ensure we continue to provide the very best care for our patients and everyone who needs us. We remain as busy as ever and ask that you ‘help us to help you’ by using the right service for your needs and when contacting or visiting the practice — treating our colleagues with the kindness and respect that they deserve.

Thank You
